Agreement for Use, Waiver, Indemnification and Release

By checking the box below I certify that I have read, understand and agree to all of the provisions of the SPC Agreement for Use, Waiver, Indemnification and Release document, and I agree to pay any and all event charges and security deposits, secure and maintain liability insurance naming Sycamore Presbyterian Church as an additional insured for the duration of the event as well as be bound by all other caveats contained therein.  These provisions also include and bind the organization which I represent by making this application and which is named previously as the "Sponsoring Organization."
